Type of inspection: Complaint Date(s) of inspection and time the licensing inspector was on-site at the facility for each day of the inspection: An on-site inspection related to a complaint was completed on January 07, 2026. The Acknowledgement of Inspection form was signed and left at the facility for each date of the inspection. A complaint was received by VDSS Division of Licensing on December 12, 2025, regarding allegations in the area(s) of: ADMINISTRATION AND ADMINISTRATIVE SERVICES, RESIDENT CARE AND RELATED SERVICES Number of interviews conducted with staff: 3 Additional Comments/Discussion: Video footage of the incident involving the resident identified in the complaint was reviewed. It shows the resident identified in the complaint appearing to be asleep in their wheelchair in a common area of the facility. Another resident appeared to trip on the wheelchair of the resident identified in the complaint, causing them to loose their balance and fall on the resident identified in the complaint, causing the resident identified in the complaint to be dislodged from their chair. The video footage shows the incident appears to be accidental. The evidence gathered during the investigation did not support the allegation(s) of non-compliance with standard(s) or law.
