Type of inspection: Complaint Date(s) of inspection and time the licensing inspector was on-site at the facility for each day of the inspection: An on-site inspection related to a complaint was completed on January 07, 2026. The Acknowledgement of Inspection form was signed and left at the facility for each date of the inspection. A complaint was received by VDSS Division of Licensing on December 12, 2025, regarding allegations in the area(s) of: ADMINISTRATION AND ADMINISTRATIVE SERVICES, RESIDENT CARE AND RELATED SERVICES Number of interviews conducted with staff: 2 Additional Comments/Discussion: Prior to the complaint being received, the provider notified the Department of an gastrointestinal outbreak affecting several residents. The provider reported they were coordinating with the local Virginia Department of Health and implementing their infection prevention policies and procedures. The provider?s infection prevention policies and procedures were reviewed. It was also determined that the infection was contained to the assisted living wing of the facility, and no residents who reside in the memory care of the wing were infected. It was also determined that while residents were encourage to quarantine in their rooms, they were not prohibited from leaving their rooms. This allowed them continual access to all areas of the facility, to include hydration stations. In addition, those residents who were asked to quarantine, had their meals brough to them. These meal deliveries, in addition to routine checks by staff, and medication administration, afforded multiple staff members at multiple times of the day to assess residents needs. The evidence gathered during the investigation did not support the allegation(s) of non-compliance with standard(s) or law.
