Comments:
A Subsidy Health and Safety inspection was conducted on 05/14/2018 to review supplemental health and safety requirements. An unannounced monitoring inspection was conducted today from 10:00am-2:30pm. There were 77 children (2-5yrs.) directly supervised by 16 staff. The physical plant, 6 staff records, 7 children?s records, medications, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed participating in group play, playing in centers which included dramatic play, blocks, sand tables, and small manipulative toys. Also children were learning about shapes, playing in the indoor playroom and outside. There was an abundant supply of materials for the children. The center was clean and organized. Areas of non-compliance are identified on the Violation Notice. A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection requirements and are identified on the violation notice. Description: Based on record review, not all staff completed the Virginia Preservice Training for Child Care Staff within 90 days of employment. Evidence: Staff#1, #2 and #3 did not have documentation to verify that they had completed the Preservice Training for Child Care Staff within 90 days of employment. Staff#1 was hired on 01/04/2010, Staff#2 was hired on 01/02/2018, staff#3 was hired on 08/21/2017.

Plan of Correction: We will locate the documentation, if unable to locate the documentation, staff will retake training.
