A Subsidy Health and Safety inspection was conducted on May 15, 2018 to review supplemental health and safety requirements. The inspectors initiated the inspection at 12:30 pm and concluded the inspection at 2:30 pm. The Owner/ Director was in attendance during the inspection. Upon the inspectors' arrival children (Infants) in the main building were observed being rocked and listening to music. Diapering was conducted with hand washing observed. Infants had time on the floor playing with soft toys. Infants were also observed being fed bottles. Staff interacted with the infants in a positive manner. Children in the preschool building were observed napping. Soft music was played while staff sat with children. All areas of the center were inspected to include classrooms and bathrooms of both buildings. The facility's vehicles were not on site for inspection. The facility had the following required information posted: the license, last inspection notice, emergency numbers, evacuation map/procedures, daily schedules, and allergy list. The first aid kits for each building were checked and found complete. Non-medical emergency supplies were also inspected. The inspector interacted with staff and children in care.
A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection requirements and are identified on the violation notice.

Three staff records and seven children's records were reviewed.
Staff have current CPR, First aid and daily health observation training and Virginia pre-service training.
Medications are not being administered. There are no staff with current MAT certification.
The most recent evacuation drill was documented on 04/18/2018. The two required shelter-in-place drills were completed on 02/28/2018 and 04/18/2018. A lock down was documented on 02/28/2018.
Today's Ratios Observed:
Infants: 4 infants with 1 staff
Toddlers 9 children with 1 staff (napping)
Pre-school: 6 children with 1 staff (napping)

Standard #: VENDSUB-000-006-B-11
Description: Based on review of children's records, the facility did not obtain a written allergy care plan for each child with a diagnosed food allergy, to include instructions from a physician regarding the food to which the child is allergic to and steps to be taken in the event of a suspected or confirmed allergic reaction; Evidence: 1. Child #6, diagnosed by a physician with a food allergy did not have an allergy care plan on record. 2. Child #7, diagnosed by a physician with a food allergy did not have an allergy care plan on record.

Plan of Correction: Per the owner: I just texted the parent requesting that she send me the allergy action plan for the two children. I will talk to her in person tonight about getting the allergy plans.
