Comments:
An unannounced monitoring inspection was conducted today, May 15, 2018 to determine the facility's compliance with licensing standards. Most but not all standards were reviewed. The inspectors initiated the inspection at 12:30 pm and concluded the inspection at 2:30 pm. The Owner/ Director was in attendance during the inspection. Upon the inspectors' arrival children (Infants) in the main building were observed being rocked and listening to music. Diapering was conducted with hand washing observed. Infants had time on the floor playing with soft toys. Infants were also observed being fed bottles. Staff interacted with the infants in a positive manner. Children in the preschool building were observed napping. Soft music was played while staff sat with children. All areas of the center were inspected to include classrooms and bathrooms of both buildings. The facility's vehicles were not on site for inspection. The facility had the following required information posted: the license, last inspection notice, emergency numbers, evacuation map/procedures, daily schedules, and allergy list. The first aid kits for each building were checked and found complete. Non-medical emergency supplies were also inspected (see violation notice). The inspector interacted with staff and children in care.


Three staff records and seven children's records were reviewed.
Staff have current CPR, First aid and daily health observation training and Virginia pre-service training.
Medications are not being administered. There are no staff with current MAT certification.
The most recent evacuation drill was documented on 04/18/2018. The two required shelter-in-place drills were completed on 02/28/2018 and 04/18/2018. A lock down was documented on 02/28/2018.
Today's Ratios Observed:
Infants: 4 infants with 1 staff
Toddlers 9 children with 1 staff (napping) (additional staff on hand were observed breaking for lunch on site)
Pre-school: 6 children with 1 staff (napping)
Per the owner, today's lunch consisted of turkey wraps, corn, oranges and milk (see violation notice).

Fire Inspection: 11/28/2017 Health Inspection 04/24/2018
Violations cited during the previous inspection were checked for corrections. There was one repeat violation cited during today's inspection. If you have questions regarding this inspection, please contact the licensing inspector at 804-662-9758.

Standard #: 22VAC40-185-420-E-3
Description: Based on review of children's records, the facility did not obtain annual documentation of parent confirmation that the required information in the child's record is up to date. Evidence: 1. The record of child #3, enrolled 09/19/2016, did not have annual documentation that the parent had confirmed the child's record was up to date. 2. The record of child #4, enrolled 01/30/2017, did not have annual documentation that the parent had confirmed the child's record was up to date.

Plan of Correction: Per the owner: I will have parents review and acknowledge by tomorrow.

Standard #: 22VAC40-185-450-A
Description: Based on observation, the facility did not ensure cots used by children during the designated rest period had linens consisting of a top and a bottom cover. Evidence: Six cots occupied by sleeping children did not have a bottom linen.

Plan of Correction: Per the owner: I will make sure to let staff know to provide a top and bottom cover if parents do not provide one.

Standard #: 22VAC40-185-540-E
Description: Based on observation, the facility did not ensure a working, battery operated radio was in each building used by children. Evidence: 1. The radio located in the main building did not have batteries and therefore was not operable. 2. The radio located in the preschool building did not have batteries and therefore was not operable. 3. Extra batteries were on site however they were not the correct size for the two radios.

Plan of Correction: Per the owner: I will get batteries this evening and make sure to keep a supply of all sizes.

Standard #: 22VAC40-185-560-F-4
Description: Based on observation, the facility did not post a menu listing foods to be served for meals and snacks during the current one week period. Evidence: The only menu posted for the current week was for one day, May 14, 2018.

Plan of Correction: Per the owner: I will make sure the weekly menu is preprinted for the week and will post this afternoon.
