Thank you for your assistance during this unannounced monitoring inspection. This on-site visit was conducted on May 16, 2017 from 11:00 a.m. through 3:15 p.m. There were 205 children in the care of 43 staff. Ten staff and ten children's records were reviewed and updated. The medication and medication paperwork was reviewed for five children. Areas of non-compliance may be found on the violation notice. Areas observed were lunch, hand washing/bathroom procedures, infant feeding and infant nap, transition, music and movement and educational activities. The interactions between the children and staff were observed as appropriate and the children see comfortable with the daily routine and schedule. Standard #: 22VAC40-185-60-A Description: Based on record review, the center failed to ensure two children had documentation of their physician's phone number. Evidence: 1. Ten children's records were reviewed. 2. Child #1 and Child #8 did not have documentation of a phone number for their physician. Plan of Correction: We will contact the parent and document the physician's phone number for Child #1 and Child #8.
Standard #: 22VAC40-185-280-G Description: Based on observation, the center failed to ensure one hazardous substance was not labeled with its contents. Evidence: 1. One spray bottle of bleach water was observed ion a shelf above the diapering area. 2. The spray bottle of bleach water was not in it's original container. The spray bottle was not labeled with it's contents. 3. The staff confirmed the spray bottle of bleach water was not labeled. Plan of Correction: Staff will be reminded that bleach water needs to be labeled when made each day.
