Discussed the following with the director: - areas of the playground which require resilient surfacing - questions regarding the annual fire inspection

An unannounced monitoring inspection was completed today. The physical plant, 6 staff files, 5 children's files, staff training, children's activities, medications, emergency procedures, drills and supplies were inspected between 12:15- 2:15pm. The outside areas were not inspected due to the weather conditions. There were 40 children under the supervision of 8 staff. During the inspection, children were observed coming back to the center from having gone on a field trip to Glen Echo, cleaning up for and eating lunch, listening to story time, enjoying free play, working on multiplication and resting during nap time. The children bring in their lunches from home each day they are in attendance. The interactions between the children and staff were seen to be positive and constructive in nature. Description: Based on file review, 1 out of 6 staff records does not have a TB test/screening completed within two years from the date of the previous TB test/screening. Evidence 1. Staff #2's most recent TB test/screening was completed on 3/15/16.

Plan of Correction: This will be completed.

Standard #: 22VAC40-185-70-A
Description: Based on file review, 1 out of 6 staff records does not have all required information. Evidence 1. Staff #1 does not have documentation that two or more references as to character and reputation as well as competency were checked before employment began; written information to demonstrate that they individual possesses the orientation training required by the job position.

Plan of Correction: The director will ask staff #1 for 2 letters of recommendation. The orientation training form will be completed and kept in the file.

Standard #: 22VAC40-185-240-A
Description: Based on file review, 1 out of 6 staff records does not have documentation of having received all required training by the end of their first day of assuming job responsibilities. Evidence 1. Staff #1 does not have verification of having received training recognizing child abuse and neglect and the legal requirements for reporting suspected child abuse as required by the Code of Virginia.

Plan of Correction: Staff #1 will complete this training.

Standard #: 22VAC40-191-40-D-1-B
Description: Based on file review, 1 out of 6 staff records does not have a search of the central registry (SCR) completed before 30 days of employment at the facility ends. Evidence 1. Staff #1 began working at the center in January 2018. The request was sent, but there was no follow up with the department after 30 days had lapsed. There is still no findings in the record.

Plan of Correction: The Director will follow up with the department.

Description: Based on file review, 1 out of 6 staff records does not have a sworn disclosure statement (SDS) completed before three years since the last checks were done. Evidence 1. Staff #6's most recently completed SDS was on 9/23/13.
