Technical Assistance:
Inspector and director discussed electronic children's files and accessibility and medication.

Comments:
An unannounced monitoring inspection was conducted today from 10:30am-1:00pm. There were 85 children directly supervised by 11 staff. The physical plant, 5 staff records, 5 children?s records, medications, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed participating in group play, playing outside, engaged in individual work using practical life items, counting, completing puzzles, writing, and working with a reading specialist. Hand-washing procedures were also observed. There was an abundant supply of materials for the children. The center was clean and organized. Areas of non-compliance are identified in the Violation Notice. Violations:
Standard #: 22VAC40-185-160-C
Description: Based on record review, the center did not ensure all staff provided a tuberculosis screening every two years. Evidence: Staff A did has a tuberculosis screening on file dated 9/30/2015 and does not have a most recent screening available.

Plan of Correction: We will make sure the TB test is completed.

Standard #: 22VAC40-185-280-B
Description: Based on observation, hazardous substances such as cleaning materials were not kept in a locked place using a safe locking method. Evidence: In Room 3, the cabinet above the sink was not locked and contained cleaning and disinfecting materials.

Plan of Correction: Upon inspection the cabinet was locked.

Standard #: 22VAC40-185-520-D
Description: Based on observation, insect repellent was not kept in a locked place inaccessible to children. Evidence: In Room 2, one insect repellent was on top of the children's cubbies accessible to children.

Plan of Correction: Insect repellent will be put away into an upper cabinet.

Standard #: 63.2-1720.1-B-3
Description: Based upon documentation reviewed, an out of state child abuse and neglect result was not obtained. Evidence: One of the five staff records reviewed states that Staff B has lived out of the state of Virginia in the preceding 5 years, and no out of state child abuse and neglect results were obtained (Staff B; DOH 07/2017).

Plan of Correction: We will submit the out of state check.
