Comments:
An unannounced monitoring inspection was conducted on May 17, 2018. Upon arrival to the facility, during the aftercare portion of the program, there were two staff present with 15 children. Children were doing various table top activities, handwashing and having snack which included mozzarella sticks and tomatoes. Five children's records were reviewed on this date. Four medications were reviewed. Licensing Inspector reviewed supervision, activities, equipment, nutrition, children's records, emergency evacuation and shelter-in-place drill documentation, fire and health inspection reports.

Standard #: 22VAC40-185-60-A
Description: Based on record review, in two of five children's records reviewed, the licensee did not ensure that each child's record was documented with all required information. Evidence: During the insepction conducted on May 17, 2018, the following information had not been documented in each child's record: Child #2-work phone number for both parents Child #3-place of employment and work phone number for one parent

Plan of Correction: Site Supervisor will speak to the parent to have them corrected.

Standard #: 22VAC40-80-120-E-2
Description: Based on observation and inspection of the facility, the licensee did not ensure that the findings of the most recent inspection of the facility were posted. Evidence: During the inspection conducted on May 17, 2018, the findings of the inspection conducted on 3/29/18 were not posted in the facility. The facility had the findings of an inspection done on 9/19/17 posted.

Plan of Correction: Site Supervisor will contact her Supervisor to let her know that it was missing. The inspection from today will be posted.
