Comments:
The licensing inspector conducted an unannounced renewal inspection on Wednesday, May 16, 2018 from approximately 11:35 am to 4:16 pm to determine the center's compliance with licensing standards. During this inspection, the center's owner/director was present and accompanied and assisted the licensing inspector throughout this inspection. The children were observed by the licensing inspector in the various classroom settings. The census for today's inspection consisted of 14 children with four staff present. During this inspection, the children in care were observed by the licensing inspector during napping. The center's bathrooms, kitchen, designated classrooms, indoor areas and equipment were inspected by the licensing inspector. The outdoor playground areas were not inspected due to inclement weather. The required posting were reviewed by the inspector. The following information was reviewed: daily attendance, emergency supplies, injury reports, evacuation and shelter-in-place drills, liability insurance policy, allergy list, and emergency preparedness plan. There were four children?s records and three staff's records reviewed during this inspection. The center's first aid kit and emergency supplies were inspected. Medication is not being administered at this time. The licensing inspector completed observations of parent pick ups, and school aged children entering the center accompanied by staff. The Department received a complete renewal application and annual fee from the center. Standard #: 22VAC40-185-60-A
Description: Based on observation and interview with staff, the center did not ensure to maintain a separate record for each child enrolled. Evidence: During the inspection on May 16, 2018, Child#5's record was not found at the center to be reviewed by the inspector. Interview with staff confirmed that child's record was not available.

Plan of Correction: Parent will be requested to return record to the center.

Standard #: 22VAC40-185-280-B
Description: Based on observation at the center, the center did not ensure hazardous substances are kept in a locked place using a safe locking method that prevents access by children. Evidence: During the inspection on May 16, 2018, there was a bottle of hand sanitizer labeled warning and the statement "keep out of reach of children" located in the Toddler classroom on the desk.

Plan of Correction: Staff will refrain from bringing personal hazardous substances in the building.

Standard #: 63.2-1720.1-B-2
Description: Based on review of three staff records and the interview with staff, the center did not have an Office of Background Investigations (OBI) fingerprinting determination letter prior to a staff beginning employment. Evidence: During the inspection on May 16, 2018, staff 1's record (date of hire: 2/16/18) fingerprinting determination letter was dated March 23, 2018. Interview with staff confirmed staff worked prior to receipt of the determination letter. The staff time card revealed dates of March 19, 2018 and March 20, 2018 in which the staff worked at the center.

Plan of Correction: Further new hires will complete fingerprinting prior to start date.
