Comments:
A Subsidy Health and Safety inspection was conducted on Wednesday, May 16, 2018 from approximately 11:35 am to 4:16 pm to review supplemental health and safety requirements. During this inspection, the center's director was present and accompanied and assisted the licensing inspector throughout this inspection. The children were observed by the licensing inspector in the various classroom settings. The census for today's inspection consisted of 14 children with four staff present. During this inspection, the children in care were observed by the licensing inspector during napping. The center's bathrooms, kitchen, designated classrooms, indoor areas and equipment were inspected by the licensing inspector. The outdoor playground areas were not inspected due to inclement weather. The required posting were reviewed by the inspector. There were four children?s records and three staff's records reviewed during this inspection. The center's first aid kit and emergency supplies were inspected. Medication is not being administered at this time. The licensing inspector completed observations of parent pick ups, and school aged children entering the center accompanied by staff. A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection requirements and are identified on the violation notice. Standard #: VENDSUB-000-006-B-11
Description: Based on observation and interview with staff, the center did not ensure to obtain a written allergy plan for each child with a diagnosed food allergy. Evidence: During the inspection conducted on May 16, 2018, Child#1's record (start date 03/13/18) was reviewed and a food allergy of seafood was identified but there was no written allergy plan found in the record. Interview with staff confirmed there was no written allergy care plan at the center for the child.

Plan of Correction: The Director will request the parent to obtain required written allergy plan from the child's physician.

Description: Based on review of children's record, the center did not ensure all requirements for the parent's place of employment was obtained. Evidence: During the inspection on May 16, 2018, a review of Child#2 's and Child#3's records were completed and the parents employment address was missing.

Standard #: VENDSUB-000-012-E
Description: Based on review of staff records, the center did not ensure completion of CPR and first aid training of staff as required within 30 days of date of employment. Evidence: During inspection on May 15, 2018, Staff#3's record (start date 4/04/18) was reviewed and there was no CPR and first aid certification found.

Plan of Correction: The staff training has been scheduled for June 2, 2018 9:00 am until 2:00 pm.

Standard #: VENDSUB-000-032-A
Description: Based on observation of emergency drills log and interview with staff, the center did not ensure to practice the minimum emergency response drills required. Evidence: During the inspection on May 16, 2018, there was no record of a lockdown procedure drill completed at the center. Interview with the staff confirmed that a lockdown drill was practiced for 2017, however there was no record available for the inspector to verify. Lockdown procedures shall be practiced at least annually.
