Comments:
Today's visit was routine monitoring inspection. the inspection began at 1:00pm and concluded at 3:40pm. There were forty-seven children in care with ten direst care staff. During the inspection all five classrooms were viewed, nap time was viewed, all three medications were viewed, emergency supplies were viewed, three out of thirteen staff records were viewed, six out of sixty children's records were reviewed, five injury reports were reviewed, and some policies and procedures were reviewed. A violation with regards to finger print based background checks was issued and follow-up with the inspector is required within ten days of today's inspection. Violations:
Standard #: 22VAC40-185-160-A
Description: Based on the inspector's review of three out of thirteen staff records, the center failed to ensure a recently hired staff member had a completed TB screening within twenty-one days of hire. Evidence: The record for staff member #2 has a documented start date of 11/6/17 and the TB screening dated 1/23/18. The TB screening after twenty-one days of hire.

Plan of Correction: All staff in the future will be required to complete a TB screening within twenty-days of hire.

Standard #: 63.2-1720.1-B-2
Description: Based on the inspector's review of three out of thirteen staff records, the center failed to ensure a portion of the staff had the results of a finger print based based background check before being employed. Evidence #1: Staff member #1 has a documented date of hire of 5/7/18 and was observed working in the center the day of the inspection. The staff member does not have the results of a finger print based background check. Evidence #2: Staff member #3 has a documented date of hire of 5/14/18 and does not have the results of a finger print based background check.

Plan of Correction: Both staff will submit to finger print based background check within the week. In the future staff will not be hire until finger print based background checks are returned.
