Technical Assistance:
Fingerprinting shall be conducted for all staff no later than September 30, 2018. Potential new staff cannot be hired until the fingerprinting is returned to the center with an appropriate finding. Background checks (search of the central registry, sworn disclosure statements, and fingerprinting checks) are valid for five years from the date of the original check. For staff who have lived outside the state of Virginia in the past five years, an out of state CPS (search of the Central registry check) must be conducted in that state(s).

Comments:
An unannounced monitoring inspection was conducted with the center director and center staff. There were 27 preschool children (between ages 2 and 5) and 7 school age children present with five staff actively supervising. The children were enjoying snacks prepared from home outside on the picnic tables and playing on the outdoor equipment. The last health inspection was conducted 2/15/2018. The last fire inspection was conducted 8/21/2017. Two emergency medications for one child were reviewed; there are four staff current in Medication Administration Training. Five children's files and two staff files were reviewed - please see Supplemental Summary for missing items.

Violations:
Standard #: 22VAC40-185-500-B
Description: Based on observation, the facility failed to have leakproof, foot operated diaper disposal system (or used in such a way that neither the staff member's hand nor the soiled diaper touches an exterior surface of the storage system during disposal). Evidence: the pull up diapers in the twos room are disposed in an open trash can, without a lid.

Plan of Correction: Facility will use a foot operated trash can for diaper disposal.

Standard #: 22VAC40-185-510-C
Description: Based on review of medication, the facility failed to have a system in place to prevent the use of outdated medication. Evidence: the expiration date on the emergency medication for one child stated 4/2018. The box that housed the medication stated the expiration date was 8/2018.

Plan of Correction: Center will keep the emergency medication on site until parent brings an updated one. In the future, staff will review the expiration dates on the actual medication.

Standard #: 22VAC40-191-40-D-3-D
Description: Based on review of two staff records, the facility failed to have a repeat sworn disclosure statement for all staff within five years since the date of the last sworn statement. Evidence: Staff B did not have a current sworn disclosure statement (original signed 12/16/2011).

Plan of Correction: Staff B will sign a new sworn disclosure statement.
