Comments:
An unannounced monitoring inspection was conducted today, May 21, 2018 to determine the center's compliance with licensing standards. The inspector arrived at 11:30 am and concluded the inspection at 1:30 pm. Upon the inspector's arrival children were preparing for lunch time. Children had bathroom breaks and hand washing was conducted along with cleaning and sanitizing of the tables. Children provide their own meals which were labeled with names and today's date. Staff sat with children and interacted with them in a positive manner. After lunch children had a nap time. Soft music was played in the back ground. The toddler class and the two year old class combined for nap time. All areas of the center were inspected to include classrooms, the indoor climbing equipment, bathrooms, and two outdoor playgrounds. The classrooms have a variety of learning centers that consisted of science, books, housekeeping, manipulatives, puzzles, art and blocks. Classrooms had age and stage appropriate materials and were decorated with children's art work. Children had been incorporating the garden theme into their curriculum. Children have planted flowers and staff stated that children water them daily. The center's first aid kit and non-medical emergency supplies were inspected. The license, daily schedule, emergency evacuation maps, emergency numbers and various parent information was posted. The inspectors interacted with staff and children during the inspection.

Five staff records and five children's records were reviewed.
Per the Director, medication is not being administered at this time. Multiple staff have current MAT, CPR, First aid and daily health observation training.
The last evacuation drill was conducted on 04/23/2018. The two required shelter-in-place drill for the 2017 were documented on 04/06/2017 and 11/29/2017
Fire Inspection: 02/27/2018 Health Inspection: 05/24/2017 (per the director, an inspection has been scheduled)

Today's ratio's observed :
Toddler: 5 children with 1 staff
Two year old: 7 children with 2 staff.
Three- four year old: 14 children with 2 staff.

Violations:
Standard #: 22VAC40-185-130-A
Description: Based on review of children's records, the facility did not obtain documentation that each child had received the immunizations required by the State Board of Health before the child attended the center.

Evidence:

Child #1, enrolled 09/11/2017, had documentation of immunizations that were dated 09/28/2017 exceeding the first day of enrollment.

Plan of Correction: Per the Director: we will get documentation prior to the start date.

Standard #: 22VAC40-185-210-A
Description: Based on interview and review of staff records, the facility did not ensure staff assigned as program leaders met program leader qualifications. Evidence: 1. Staff #1 was observed alone and caring for five toddlers in the toddler classroom. 2. The director stated that staff #1 had been acting as program leader sense March of 2018. 3. Staff #1 did not have the qualifications of program leader.

Plan of Correction: Per the Director: I will have staff lead qualified within 30 days.
