Comments:
A Subsidy Health and Safety inspection was conducted on 05/22/18 to review supplemental health and safety requirements. There were 60 children in care of 5 staff members. The children were observed eating snack, washing hands, playing with table top toys and playing outside. A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection requirements and are identified on the violation notice. Standard #: VENDSUB-000-012-C
Description: Based on record review and staff interviews, staff has not completed the Virginia Preservice Training within 90 days of this agreement being effective. Evidence: 1. Staff # 2's record did not have documentation of the completed Virginia Preservice training. Staff # 2 was hired on 10/31/2016. 2. Staff # 3 stated that she had completed the training but the certification was available for review during the inspection. The certificate was not in the file.

Plan of Correction: Staff will complete the training and bring it to be filed.

Standard #: VENDSUB-000-012-E-1
Description: Based on record review, center did not maintain current certification in cardiopulmonary resuscitation (CPR) for staff. Evidence: 1. Staff # 1's CPR certification expired on 8/2016. The renewed CPR certification was not on file.

Plan of Correction: I will file the certification as soon as I get it from the office.

Standard #: VENDSUB-000-012-E-2
Description: Based on record review, center did not maintain current certification in first aid for staff. Evidence: There was no first aid certification available on file for Staff # 1.

Plan of Correction: I will obtain the certification from the office and file it.
