Technical Assistance:
22 VAC 40-185-130 (immunizations for children), 22 VAC 40-185-330 (play areas) and 22 VAC 40-185-560 (nutrition and food services) were discussed with the center owner and director during the inspection.

Comments:
An unannounced monitoring inspection was conducted from 10:00am until 1:00pm. At the time of the tour, there were 88 children in care with 17 staff present. A sample of 9 children's records and 8 staff records were reviewed. Children were observed participating in morning learning activities, eating lunch and resting quietly during nap time. Handwashing and restroom procedures were also observed. Medication, first aid and emergency supplies, documentation of emergency practice drills, children's injury reports and center postings were reviewed. Two violations in the area of administration are identified on the violation notice and were discussed with the center owner and director during the exit interview.

Violations:
Standard #: 22VAC40-185-160-A
Description: Based on record review and interview, the center did not ensure that each staff member shall submit documentation of a negative tuberculosis screening completed within 12 months prior to or 21 days after employment. Evidence: 1. Staff #1 has a documented date of hire of 3/26/18. The record for staff #1 contains documentation of a tuberculosis screening dated 9/16/16, which is more than 12 months past the date of hire. 2. The director confirmed that the tuberculosis screening for staff #1 is more than 12 months past the date of hire.

Plan of Correction: The center responded with the following: Staff #1 has gone and gotten her TB test and gets her results read today so that will be in her file as of today.

Standard #: 22VAC40-185-60-A
Description: Based on record review and interview, the center did not ensure that children's records contain all required components. Evidence: 1. The record for child #1 contains one address for emergency contacts, where two are required. 2. The record for child #2 contains no addresses for the two required emergency contacts. 3. The director confirmed that the records for child #1 and child #2 do not contain the addresses for the two emergency contacts.

Plan of Correction: The center responded with the following: Child #1 and #2 brought in their new emergency contact form with addresses on them.
