Comments:
A Subsidy Health and Safety inspection was conducted on 5/2/2018 to review supplemental health and safety requirements. Today an unannounced monitoring inspection was conducted between the hours of 2:20pm and 5:00pm. I observed 24 children with 3 staff members. Ratios and supervision were in compliance. Children were observed arriving to the program, washing hands, eating snack, playing outdoors, doing homework, reading, using the center's computers and ipads, and playing table games. Staff were actively engaged with the children and assisting them with tasks as needed. The center was found to be clean, safeguarded appropriately, and adequately supplied with equipment and supplies. Required staff training documents were on-site. Emergency preparedness supplies in order. Required documents posted. Questions about this inspection may be directed to: maria.soto@dss.virginia.gov A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection requirements and are identified on the violation notice. Standard #: VENDSUB-000-012-E-1
Description: Based on record review and interview, two staff member did not have a current certification in cardiopulmonary resuscitation (CPR). Evidence: The records of staff#3 did not contain documentation of a current CPR certification. According to the subsidy vendor agreement, staff is required to have a current CPR certification within 30 days of beginning employment.

Plan of Correction: Staff will get the certifications.

Standard #: VENDSUB-000-012-E-2
Description: Based on record review and interview, one staff member did not have a current first aid certification. Evidence: The record of staff#3, #4 and #5 did not contain documentation of a current first aid certification. According to the subsidy vendor agreement, staff is required to have a current first aid certification within 30 days of beginning employment.

Plan of Correction: Staff will get the certifications.

Standard #: VENDSUB-000-032-A
Description: Based on record review, lockdown drills were not practiced annually. Evidence: There was no documentation of a lockdown drill during the inspection.

Plan of Correction: We practiced the drill but we can't locate the form where we documented it.
