22VAC40-191 Background Checks (22VAC40-191)
An unannounced monitoring inspection was complete today. The physical plant, 5 staff files, 5 children's files, annual fire inspection, staff training, child nutrition, children's activities, medications, emergency procedures, drills and supplies were inspected between 3-4:40. There were 45 children under the supervision of 10 staff. During the inspection, children were observed cleaning up for and eating afternoon snack, playing outside on the playground, having diapers changed and enjoying free play. The children bring in their lunches and snacks daily from home. The interactions between the children and staff were seen to be positive and constructive in nature. There are currently 3 staff certified in MAT (medication administration training). Should you have any questions, please contact me at email@example.com.
Standard #: 22VAC40-185-160-C Description: Based on file review, 1 out of 5 staff records did not have a TB test/screening completed within the past two years. Evidence 1. Staff #1's most recent TB test/screening was completed on 4/22/16. Plan of Correction: This has been scheduled to be completed this weekend.
Standard #: 22VAC40-185-270-A Description: Based on observations, there was an area of the center which was not kept in a safe condition. Evidence 1. In the 16-24 months old classroom, there was an area on the window sill which had peeling paint. Plan of Correction: The Director will have this fixed.
Standard #: 22VAC40-185-510-C Description: Based on file review, 2 children did not have current medication authorizations for medications. Evidence 1. Child #6 requires an epi-pen as an an emergency medication. The medication authorization expired on 4/9/18. 2. Child #7 requires benedryl as an emergency medication. The medication authorization expired on 3/21/18. Plan of Correction: The director gave the parents of child #6 a form to be completed, but it has not been returned. Both parents will be asked to bring a new form into the center.
