An unannounced monitoring inspection was conducted from 7:45 am-9:00. There were a total of 28 children with 2 staff members present. Children were observed engaged in group activities and riddle games. A selection of staff trainings, children records, medications and other documents were reviewed. Standard #: 22VAC40-185-140-A Description: Based on record review, the center did not ensure that children's files contained a physical examination within 30 days of enrollment. Evidence: The record for Child #1, enrolled 8/30/17 was missing a physical examination. Plan of Correction: We will get the physical from the office.
Standard #: 22VAC40-185-240-D-1 Description: Based on record review and staff interview, the center has not ensured that a MAT trained staff was in attendance whenever a child has medication. Evidence: There were several children in attendance with medications, the two staff caring for children during the before school program did not have MAT training. Plan of Correction: We will schedule staffing so that someone with MAT training is always in attendance. A MAT trained staff will be in attendance during the after school program.
