Comments:
The licensing inspector conducted an unannounced complaint inspection in response to complaints that were received by the licensing department on April 10, 2018 and April 13, 2018 regarding allegations of the center being over ratio, background checks not being conducted for staff and inappropriate conversations occurring in front of the children in care. The inspection was initiated on May 10, 2018 and completed on May 17, 2018. Interviews with staff were completed, observations were made in the center, and documentation was reviewed. The preponderance of the evidence gathered during the investigation supports part of the allegation; therefore, the complaint is determined to be valid. In addition to the original complaint, there were also additional violations found during the inspection. See the violation notice for more details. Complaint related: No
Description: Based on observation and staff interview on 05/10/2018, the center did not ensure that children under 10 years of age always shall be within actual sight and sound supervision of staff. EVIDENCE: 1. Upon the inspector's arrival there was no staff in the infant room. 2. There were four infants resting in the infant room. 3. Staff #1, who was observed in the preschool room, stated that they were supposed to be in the infant room.

Complaint related: Yes
Description: Based on observation and staff interview on 05/10/2018, the center did not ensure that during the designated rest period the ratio of staff to children was double the required number of children to each staff. EVIDENCE: 1. There were 22 children to one staff present in the preschool room during the rest time on that day. 2. Staff #2 stated that the youngest child in that group of children was two years old. 3. Effective June 1, 2006 the ratio for children two years old is one staff member for every eight children 4. There were only two staff present at the center Staff #1, who was in the infant room and Staff #2, who was in the preschool room.

Plan of Correction: Had a staff meeting on supervision and ratios on 05/10/2018. Provided inspector with agenda and staff agreement that was reviewed.

Complaint related: No
Description: Based on observation on 05/10/2018, the center did not ensure that there was at least 12 inches of space between occupied rest mats. EVIDENCE: There was less that 12 inches between the mats of the children resting in the preschool room.

Plan of Correction: Addressed in the staff meeting.
