- Technical Assistance:
New background check regulations were discussed with the director during the inspection. All staff must complete fingerprint background checks through the Office of Background Investigations (OBI) by September 30, 2018.
An unannounced monitoring inspection was conducted from 11:25am until 1:30pm. At the time of the tour, there were 40 children in care with 7 staff present. A sample of 5 children's records and 5 staff records were reviewed. Children were observed eating lunch, practicing for their end of the year program and resting quietly during nap time. Table sanitizing, hand washing and restroom procedures were also observed. First aid and emergency supplies, the emergency preparedness plan, documentation of emergency practice drills and center postings were reviewed. Medication is not being administered to children in care at this time. Two violations in the areas of administration and special care provisions and emergencies are identified on the violation notice and were discussed with the director during the exit interview.
Standard #: 22VAC40-185-160-C Description: Based on record review and interview, the center did not ensure that at least every two years from the date of the first initial screening or testing, staff members shall obtain and submit the results of a follow-up tuberculosis screening. Evidence: 1. The most recent tuberculosis screening for staff #1 is dated 7/28/15. 2. The most recent tuberculosis screening for staff #2 is dated 9/22/15. 3. The most recent tuberculosis screening for staff #3 is dated 12/4/15. 4. The most recent tuberculosis screening for staff #4 is dated 5/27/16. 5. Staff #1 confirmed that the tuberculosis screenings for herself and staff #2, 3 and 4 are over two years old. Plan of Correction: Staff will obtain updated TB screening/test and results will be placed in the record.
Standard #: 22VAC40-185-550-E Description: Based on a review of center documents and interview, it was determined the center did not ensure that a record of practice drills is maintained for one year. Evidence: 1. There was no documentation of an evacuation drill for May 2018. 2.Staff #1 stated that an evacuation drill for May 2018 was completed but was not documented in the log book. Plan of Correction: The fire drill that occurred on 5/31/18 was documented in the log book during the inspection.
