Technical Assistance:
An exit interview was conducted with the director and office manager to discuss the results of today's inspection.

Comments:
A non-mandated focused inspection was conducted today, May 18, 2018 to determine the center's compliance with licensing standards. The inspection was initiated at 10:00am and concluded at approximately 12:15pm. Upon arrival, the following ratios were observed:

18m-2yrs: 9 children/ 2 staff
3yrs-9yrs: 11 children/ 2 staff

Children were observed interacting with staff, playing with toys, using the restroom, and eating lunch. The center had the required postings displayed on the bulletin board upon entering the facility. The center's first aid kit and emergency supplies were inspected with additional supplies found. Six children's records and four staff records along with other administrative records were inspected with the majority of the required information found captured. Previous violations were inspected on this day; however due to today's inclement weather the playground and bus used for transportation were not inspected.

Violations:
Standard #: 22VAC40-185-140-A
Description: Based on the inspection of administrative records, the required documentation was not available to review.

Evidence:
During the inspection of six children's records, it was discovered child #1 enrolled on 7/24/17 and child #2 enrolled on 1/15/18 did not have documentation of the children's physical examination.

Plan of Correction: Per owner: I will request this information from the parent today and will allow them a week to provide the documentation.

Standard #: 22VAC40-185-60-A
Description: Based on the inspection of administrative records, the required documentation was not available to review.

Evidence:
During the inspection six children's records, it was discovered child #2 enrolled on 1/15/18 was missing signed parent agreements; while child #4 enrolled on 12/4/17 was missing the required custody order/documentation.

Plan of Correction: Per owner: I will get the information from the parents soon as possible.

Standard #: 22VAC40-185-70-A
Description: Based on the inspection of administrative records, the required documentation was not available to review. Evidence: Based on the inspection of three staff records, it was discovered staff #2 did not have documentation that two or more references have been checked.

Plan of Correction: Per owner: I will speak with the employee to obtain the required references. I will obtain this information within the week.

Standard #: 22VAC40-185-240-A
Description: Based on the inspection of administrative records, the required orientation documentation was not available to review. Evidence: During the inspection of three staff records, it was discovered all three staff did not have documentation of orientation training. Licensing standards require orientation training to be conducted by the end of the staff's first day of assuming job responsibilities with documentation maintained for review.

Plan of Correction: Per owner: I will make sure new employee's have orientation by the end of the first day and have a form completed.

Standard #: 22VAC40-191-40-D-1-C
Description: Based on the inspection of administrative records, the required documentation was not maintained.

Evidence:
During the inspection of staff records, it was discovered staff #4 did not have documentation of the central registry background check previously submitted.

Plan of Correction: Per owner: I will contact the proper agency to obtain a copy of the background check.
