Technical Assistance:
An exit interview was conducted with the director to discuss the results of today's inspection. Consultation was given regarding updated background check regulations.

Comments:
An unannounced monitoring inspection was conducted today, June 1, 2018 to determine the center's compliance with licensing standards. The inspection was initiated at 1:48pm and concluded at 4:00pm. Upon arrival, the following ratios were observed:
Infant room: 4 infants/ 1 staff
Toddler room: 4 toddlers/ 1 staff
Two-Three year olds room: 6 children/ 1 staff
Three year olds room: 8 children/ 1 staff
Three-Four year old room: 9 children/ 1 staff
*School age children had not arrived during this inspection.

Children were observed sleeping on cots with the proper bedding, using the restroom, eating snack, playing with toys, and departing for the evening. Today's snack consisted of sun chips and fruit. Children's books, toys, and equipment were inspected and found to be age appropriate and in operable condition. All classrooms were inspected to include main hallways, children's restrooms, and the outdoor blacktop area used for outdoor play.

The center had the required postings displayed upon entering the center and in children's classrooms. The center's first aid kit and emergency supplies were inspected with additional supplies found. Five children's records and two staff records were inspected with the majority of the required information present. The following was also inspected: daily attendance, daily infant reports (2), fire/health inspections, fire/shelter-in-place drills, parent agreements, and other pertinent information.

Violations:
Standard #: 22VAC40-185-140-A
Description: Based on the inspection of children's records, the required examination was not in the record. Evidence: Based on the inspection of five children's records, it was discovered child #1 did not have a physical examination in the record.

Plan of Correction: Per director: I will call the parent to obtain a copy of the child's most recent physical.

Standard #: 22VAC40-185-270-A
Description: Based on the visual inspection of the children's classrooms, an unsafe condition was found. Evidence: During the inspection of children's classrooms, chipped paint was found in various areas on the wall (Dora the Explorer painting) in the 2 1/2 year old classroom. Licensing standards require areas and equipment of the center, inside and outside, shall be maintained in a clean, safe and operable condition. Unsafe conditions shall include, but not be limited to, splintered, cracked or otherwise deteriorating wood; chipped or peeling paint; visible cracks, bending or warping, rusting or breakage of any equipment.

Plan of Correction: Per director: I will have the wall painted as soon as possible.
