Technical Assistance:
Consultation was given on repeat violations and the need to address the issues. The license will not be issued until an out of state CPS search is returned for a board officer.

Comments:
An unannounced renewal study was conducted from 1:00-3:30. Reviewed a sample of records for staff and children, physical space, emergency preparation, training and care of children. Consulted on toys and equipment being easily available for the child minding classroom. Areas of non-compliance are identified in the violation notice. Standard #: 22VAC40-185-70-A
Description: Based on record review, one of five records for staff was incomplete. Staff #1 was missing a second reference check prior to employment. (date of hire 10/2017)

Plan of Correction: Reference checks sent to employees references.

Standard #: 22VAC40-185-270-A
Description: Based on observation, areas and equipment have not been maintained in a clean, safe and operable condition. Evidence: 1. Peeling paint is observed along the window sill in the girl's bathroom. 2. A nail was protruding from the back side of the cabinet in the child minding room.

Plan of Correction: A work order has been placed to fix the peeling paint as well as remove the screw from the cabinet.

Standard #: 22VAC40-185-280-B
Description: Based on observation, hazardous substances are not kept in aa locked place that prevents access by children. Evidence: Cleaning wipes were observed in the gym and in the child minding room as well as a nutritional mix in the child minding room in an unlocked cabinet.

Plan of Correction: Cabinet lock has been installed in the child room. Wipes are places in a locked cabinet in the gym.

Standard #: 22VAC40-185-510-A
Description: Based on medication review, a medication is missing written parent authorization. Evidence: Child #1 has an emergency medication on-site with no written parent authorization to administer the medication.

Plan of Correction: Obtain the medication authorization from the parent.

Standard #: 22VAC40-185-510-C
Description: Based on medication review, a medication for child #1 was missing a long term authorization signed by the physician. Evidence: No authorization was present for an emergency medication that was on-site longer than 10 days.

Plan of Correction: Medication authorization will be obtained by the parent.

Standard #: 22VAC40-80-120-E-2
Description: Based on observation, the most recent inspection notes for the facility were not posted. Evidence: The Violation Notice from 10/12/2017 was not posted as required.

Plan of Correction: The notes will be posted as required.

Standard #: 63.2-1720.1-B-3
Description: Based on management statement, a board officer is missing the out of state central registry form as required by the Code of Virginia. Evidence: Board Officer #1 is missing a completed out of state central registry form from Maryland.

Plan of Correction: A completed and notarized form was sent into Maryland as required to complete the background checks.
