Comments:
An unannounced complaint inspection was initiated on March 23, 2018, regarding staffing and supervision. The inspection included a review of center documentation and interviews with staff members . Based on information obtained during the inspection the preponderance of evidence does support the allegation related to staffing and supervision therefore the complaint is determined to be VALID. Areas of non compliance are noted on the violation notice.

Description: Based on interviews , the licensee did not ensure the care, protection, and guidance of the children. Evidence: On March 14, 2018, approximately between 4:50-4:55pm, the three year old classroom children were coming in from outside play ground. Staff #1 stated she did a face to name count before entering the building and was the back of line following the children into the building. Child #1's parent arrived approximately between 4:50-4:55 to pick up . Child #1's parent did not see her child in the classroom. Parent asked staff #1 where her child was. Staff #1 open the door and found child #1 and child #2 outside next to the door.

Description: Based on staff interview, the licensee did not ensure children under 10 years of age always are within actual sight and sound supervision of staff. Evidence: During the inspection conducted on March 23, 2018, staff #1 and staff #2 confirmed, on March 14, 2018, child #1 and child #2, three years of age were alone outside in a fenced area for an undetermined amount of time.
