Comments:
The Licensing Inspector conducted an unannounced complaint inspection in response to a complaint that was received by the licensing office on 5/25/2018. Interviews were conducted with staff relating to the allegations of staffing and staff qualifications. The information gathered during this investigation supports the allegations, so the complaint is determined to be valid. Please complete the columns for "Plan of Correction" and "Date to be Corrected" for each violation cited on the violation notice, and then return a signed and dated copy to the licensing office within 5 business days of receipt. Complaint related: No
Description: Based on interview, one staff member is not capable of effective oral and written communication. Evidence: 1. When the Inspector asked Staff #1 if they could speak to Staff #2, Staff #1 stated that Staff #2 would need a translator because they do not speak English. 2. When the Inspector asked the Program Director about Staff #2's English speaking skills, the Program Director confirmed that Staff #2 know's basic English words but could not carry on a conversation without a translator. The Program Director stated that Staff #2 would not be able to complete a written report if necessary.

Plan of Correction: Staff #2 will take English courses to improve oral and writing skills.

Description: Based on interview, one classroom did not have at least one staff member who met the qualifications of a program leader who was regularly present. Evidence: 1. When asked who was the lead teacher in the Baby Bunnies classroom, the Program Director stated that the program leader had resigned from their position on 2/24/2018 and that the Assistant Director was filling in while they are interviewing for a new program leader. 2. When interviewing the Assistant Director about their frequency in the Baby Bunnies classroom, they stated that they fill in when there are not enough staff in the classroom. They stated that they also send several teachers in other classrooms on their lunch breaks. 3. When interviewing Staff #3 about who is the program leader in the Baby Bunnies classroom, they stated that they do not currently have one and that the previous leader had resigned a couple of months ago.

Plan of Correction: The Assistant Director will assume full time Lead responsibilities for the Infant program until a qualified replacement is found.
