An unannounced monitoring inspection was conducted from 9:00 ? 11:00am with the admissions director and staff. There were 35 children, ranging from nineteen months to five years of age, present with five staff supervising. Children were observed participating in work time in different areas of the Montessori environment, playing outside, and eating snack. Snack today was cucumbers and almonds with water (provided by parents). Four children's records and three staff records were reviewed. The emergency drill log was reviewed. Standard #: 22VAC40-185-140-A Description: Based on a review of four children's files, the facility did not obtain documentation that each child has received a physical examination before the child's first day of attendance or within one month after attendance. Evidence: Child 1 (date of first attendance: 9/17) and Child 4 (date of first attendance: 10/11/2015) did not have documentation of a physical examination in their file. Plan of Correction: Child has an appointment for a physical.
Standard #: 22VAC40-185-160-C Description: Based on a review of three staff files, it was determined that repeat tuberculosis (TB) test/screenings were not completed every two years. Evidence: Documentation of the tuberculosis test for staff 3 is dated 6/8/15. Plan of Correction: Have documentation of updated TB.
Standard #: 22VAC40-185-40-D Description: Based on observation, the license was not posted in a place conspicuous to the public. Evidence: The license was posted inside the director's office on a back wall. Plan of Correction: Will post in a public place.

Standard #: 22VAC40-185-350-C Description: Based on observation and staff interview, it was determined that the facility is not maintaining the staff to children ratio for a regularly ongoing mixed aged group applicable to the youngest child in the group. Evidence: There were 28 children present with three staff supervising. The youngest child in the group (Child 3, date of birth: 8/29/2015) is two years old. Standard #: 22VAC40-185-500-A Description: Based on observation, the facility failed to wash children's hands with soap and running water after toileting. Evidence: after using the toilet, a toddler did not wash his hands before returning to the group area to play. Plan of Correction: Staff have been retrained on hand washing procedures.
Standard #: 22VAC40-185-550-D Description: Based on a review of documentation, the facility did not have a record of the dates of the practice shelter in place drills for the past 12 months. Evidence: The record of practice drills for 2017-18 included dates of the evacuation drills but did not include dates of the shelter in place drills. Plan of Correction: Have been conducting shelter-in-place drills but not documenting them. Will begin documenting them.

Standard #: 63.2-1720.1-A Description: Based on review of three staff files it was determined that not all staff had criminal history record (fingerprint) checks prior to their first day of employment.
