Comments:
A Subsidy Health and Safety inspection was conducted on 6/6/18 from 9:55am to 12:05pm to review supplemental health and safety requirements. At the time of entrance there were 62children ages two months to five years old in care with 14 staff. Children were observed participating in various activities in the classrooms, playing outside, eating lunch, and during rest period. Records were reviewed for six children and five staff were reviewed during the inspection. A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection requirements and are identified on the violation notice.

Violations:
Standard #: VENDSUB-000-006-B-11
Description: Based on a review of the facility allergy list and six children's records, it was determined that the vendor did not ensure that a written allergy care plan for each child with a diagnosed food allergy, to include instructions from a physician regarding the food to which the child is allergic to and steps to be taken in the event of a suspected or confirmed allergic reaction. Evidence: 1. The record for child #1 contained documentation from a physician that child #8 was allergic to peanuts. However, there was no allergy plan present for child #1 at the facility. 2. Administrative staff reviewed the record for child #1, and confirmed that neither child had an allergy plan at the facility.

Plan of Correction: The vendor responded: The parents of child #11 will be asked to provide an allergy plan that has been completed by the child's physician. All newly enrolled children with a diagnosed food allergy will have an allergy plan that has been completed by the child's physician prior to starting care.

Standard #: VENDSUB-000-012-E-1
Description: Based a review of five records, it was determined that the vendor did not ensure that all staff have a current certification in cardiopulmonary resuscitation (CPR) appropriate to the age of children in care. Evidence: 1. The record for staff #3 did not contain documentation for a current certification in cardiopulmonary resuscitation (CPR) appropriate to the age of children in care. There was a CPR certification card that expired on 4/27/18 2. Administrative confirmed there was not documentation for to demonstrate that staff #3 had a current certification in cardiopulmonary resuscitation (CPR) appropriate to the age of children in care.

Plan of Correction: The vendor responded: Staff #3 will complete a course to obtain certification in cardiopulmonary resuscitation (CPR) appropriate to the age of children in care. All new staff will complete a course to obtain certification in cardiopulmonary resuscitation (CPR) appropriate to the age of children in care within 30 days of employment

Standard #: VENDSUB-000-012-E-2
Description: Based a review of five staff records, it was determined that the vendor did not ensure that all staff have a current certification in first aid appropriate to the age of children in care. Evidence: 1. The record for staff #3 did not contain documentation for a current certification in first aid appropriate to the age of children in care. There was a first aid certification card that expired on 4/27/18 2. Administrative staff confirmed there was not documentation for to demonstrate that staff #3 had a current certification in first aid appropriate to the age of children in care within 30 days of employment.

Plan of Correction: The vendor responded: Staff #3 will complete a course to obtain certification in first aid appropriate to the age of children in care. All new staff will complete a course to obtain certification in first aid appropriate to the age of children in care within 30 days of employment.

Standard #: VENDSUB-000-032-A
Description: Based on a review of the emergency drill log, it was determined that the vendor did not ensure that the lockdown procedures are practiced annually. Evidence: 1. There was no documentation to demonstrate that the lockdown procedures were practiced during 2017. 2. Administrative staff viewed the facility's emergency drill log, and confirmed they would be unable to provide documentation during the inspection to demonstrate that a lockdown drill was completed in 2017.

Plan of Correction: The vendor responded: We will ensure that we complete a lockdown drill annually and document the drill on the emergency drill log.
