Comments:
An unannounced monitoring inspection was conducted with the director beginning at 9:40 AM and ending at 12:45PM. There were seventy-nine children in care during the inspection. The children's ages ranged between 8 weeks and 5 years old. The infants were observed sleeping in cribs, playing with soft toys, and doing "tummy time". The older children were observed being read stories by caregivers, painting, and during outside recreational time. Diaper changing, bathroom, and hand washing procedures were observed and in compliance. Ten children's records were reviewed and in compliance. Six staff records were reviewed. Seven medications were reviewed and in compliance. The emergency escape drill log was reviewed and in compliance. The most recent emergency escape drill was documented as 5/31/18. The most recent shelter-in-place drill was documented as 5/30/18. The most recent lockdown drill was documented as 3/31/17. The most recent health inspection was documented as 10/19/17. Standard #: 63.2-1720.1-A
Description: Based on a review of documentation, the center did not have an Office of Background Investigations (OBI) fingerprint-based criminal background history record determination letter prior to a staff beginning employment. Evidence: Staff A's (hired 5/22/18) record did not include an OBI fingerprint determination letter. Staff A was present during the monitoring inspection on June 7, 2018.

Plan of Correction: The staff member is scheduled for fingerprinting today (5/7/18). The staff will not return to work at the center until the fingerprint determination letter has been received by the center.
