The Licensing Inspector conducted an unannounced complaint inspection in response to a complaint that was received by the licensing office on 6/11/2018. Documents were reviewed and the director was interviewed related to the allegations that the center did not maintain a written record of a child's minor injury and supervision. The information gathered during this investigation supports the allegation that the center did not maintain a written record of a child's minor injury with all required items, so the complaint is determined to be valid. The information gathered during this investigation regarding supervision does not support the allegations, so the complaint is determined to be not valid. Please complete the columns for "Plan of Correction" and "Date to be Corrected" for each violation cited on the violation notice, and then return a signed and dated copy to the licensing office within 5 business days of receipt. Standard #: 22VAC40-185-550-M Complaint related: Yes Description: Based on interview, the center did not maintain a written record of one child's minor injury. Evidence: 1. The Center director stated she did not have a copy of the injury report dated 6/05/2018 for child A because the parent took the injury report with her on 6/11/2018. Plan of Correction: I called the parent during the inspection and asked if she could bring the injury report back to the center. If she doesn't bring it back by close of business tomorrow, I will rewrite the report and have two staff sign it.
