Comments:
An unannounced monitoring inspection was conducted from 9:00am - 11:10am. Upon arrival there were 72 children in care with 13 staff present. A sample of 2 staff and 7 children's records were reviewed. The center did not have any medications or any children enrolled at the time of inspection with any diagnosed food allergies. Children were observed participating in painting, drawing and in learning centers. Infants were observed sleeping in cribs and playing on the floor with staff.

Violations:
Standard #: 22VAC40-185-60-A
Description: Based on a record review the center did not ensure all required information was kept for each enrolled child. Evidence: 1. Child #2 (dob 07/23/09) (doe 09/05/17) did not have a complete address for 1 of the 2 required emergency contacts.

Plan of Correction: The following corrective action was provided by the program director during the exit interview: The director will ask the parent today to include the full address on the enrollment form

Standard #: 22VAC40-185-270-A
Description: Based on observation the center did not ensure areas and equipment of the center were shall be maintained in safe condition. Evidence: 1. In the infant room the rocking chair cushion had a tear about an inch wide, exposing the inside foam. 2. In the school-age room the stage was torn and the plywood underneath was exposed and splintered.

Plan of Correction: The following corrective action was provided by the program director during the exit interview: The program director ordered a new rocking chair for the infant room during the inspection and placed a work order to have the stage fixed.

Standard #: 22VAC40-185-290-3
Description: Based on observation the center all electrical outlets did not have protective covers. Evidence: In the infant room there were 2 electrical outlets on a powerstrip that did not have protective covers. The powerstrip was on the counter with the refrigerator.

Plan of Correction: The following corrective action was provided by the program director during the exit interview: Covers will be placed on the electrical outlets.
