Discussed postings of the license and inspection notices. Also had a conversation about procedures regarding children loading and unloading center's bus.
Today an unannounced focused monitoring Inspection took place between the hours of 3:00pm and 4:50pm. At the start of the inspection there were a total of 65 children with 6 staff; all ratios were incompliance. A total of 5 children?s files and 5 staff files were reviewed. Standard #: 22VAC40-185-140-A Description: Based on record review, each child did not have documentation of a physical exam under the director of a physician. Evidence: Child #5 file did not contain documentation of a completed physical exam. Plan of Correction: I will email the parents letting them know we need an updated form.
Standard #: 22VAC40-185-60-A Description: Based on record review, children's records did not contain all required information. Evidence: Child #5 record did not contain at least one local emergency contact that could be contacted in case of an emergency. Plan of Correction: I will email the parents requesting new emergency contacts.

Evidence: Staff #1 and Staff #2's files did not contain documentation of two completed reference checks or the name, phone number and address of someone to be contacted in case of an emergency. Staff #3's file did not contain documentation of two reference checks. Plan of Correction: This was corrected during the inspection.

Evidence: Staff #1 and Staff #2 files did not contain documentation of receipt of orientation. Plan of Correction: This was corrected during the inspection.
