Thank you for your time and assistance with the unannounced monitoring inspection conducted today, June 13, 2018. The on-site inspection was from 12:00 p.m. to 4:15 p.m. On this date, at the time of arrival, there were 28 children in the care of four staff members, and three extra staff members were available to assist, if needed. There were 36 children enrolled in the program. There were seven staff members' records reviewed for updates and five children?s records reviewed. The medications and medication paperwork were reviewed for two children. There were five staff members and three children interviewed, and four classrooms were observed. The exceptions to compliance were noted on the violation notice and the risk ratings were discussed. Snack met USDA guidelines and consisted of goldfish crackers and milk. Some items observed and reviewed during the inspection included: choice time, infant care, diaper changing procedures, hand washing and bathroom procedures, nap time, afternoon snack, dismissal procedures, required posted information, emergency evacuation, shelter-in-place drills, and lockdown drills, injury reports, first aid kit and emergency supplies, annual fire and health inspections. Standard #: 22VAC40-185-500-A Description: Based on observation and verification from staff, the center failed to wash the hands of two children after diapering. Evidence: 1. The staff in Classroom A did not wash the hands of one child after diapering. 2. The staff in Classroom B did not wash the hands of one child after diapering. Plan of Correction: Both staff members were retrained during the inspection and the director will find an additional training for the staff members.
Standard #: 22VAC40-185-510-D Description: Based on review of medication and medication paperwork and confirmation with the director, the center failed to obtain parent authorization to allow the center to administer medications to two children. Evidence: 1. There was no parent authorization for Child A allowing the center to administer the medication that was on-site. 2. There was no parent authorization for Child B allowing the center to administer the medication that was on-site. Plan of Correction: The center will contact the parents immediately and have them complete medication authorizations.
