We discussed the requirements for using children's wading pools, children's records and tuberculosis screening statements. The risk assessments for violations were provided. Also, we talked about diaper changes preparations, and procedures to ensure sight supervision of children during transitions through the multi-purpose room.
Thank you for your assistance during today's unannounced monitoring inspection conducted from 10:15 AM to 5:10 PM. Today, there were seventy-eight children, ages infancy through nine years, with twelve staff. I viewed staffing, supervision, interactions with children, equipment, activities, posted information, infant care and records, sunscreen, diaper ointment, diapering procedures, agency reports, insurance, nutrition, menu, daily routines, medication, injury reports, emergency supplies and drill records, records for eight children, ten staff records, staff qualifications, staff training, indoor and outdoor areas. We discussed your procedures for transportation, swimming permissions and walking field trips. The children have enjoyed regular activities in the adult care room. Today, one group used play dough with adults at tables. Each group had theme planned age appropriate activities and access to a variety of materials and toys. Standard #: 22VAC40-185-160-B Description: Based on a review of ten records for staff, and an interview with administrative staff, the results of the PPD negative tuberculosis screening for staff member 7 did not include all required information. Evidence: The clearance statement had a place at the bottom for the physician's signature and was not signed by the physician or designated health care provider. Plan of Correction: The program director immediately faxed the statement to the office of the health care provider and requested that a signed copy of the statement be returned to the center.
Standard #: 22VAC40-185-60-A Description: Based on a review of eight records for children, and an interview with administrative staff, there was required parent employment information missing from the records of child 1 and 5. Evidence: The section for listing the place of employment and work phone number for one parent of child 1 was blank on the registration forms, emergency contact list, and cover sheet for the health records. The phone number for a parent's written employer information was not in the record for child 5. Plan of Correction: The director immediately obtained the work phone number for a parent of child 5. The director knew the town and type of employment for child 1's parent, located the business name and phone number, and then called to verify the information that was added to the record. The administrative staff who accept enrollment paperwork will review the information to make sure all required work contact information has been provided for new children's records.
Standard #: 22VAC40-185-470-E Description: Based on observation of the use of three portable wading pools, sprinkler and water tables during outdoor water activities this morning, and interviews with staff of two age groups who conducted water play time this week, children in the two year old group who are not toilet trained have been allowed to use the portable wading pools. Evidence: Staff confirmed that on June 19, 2018, eight of twelve children in the two year old group wore "swimmy" disposable diapers and were not yet toilet trained while using the wading pools and some were sitting in the water. Plan of Correction: The administrative staff will instruct the staff who conduct outdoor water activities with children to only use wading pools with children who are toilet trained. The requirement for children to be toilet trained if they use wading pools will be reviewed at the start of each summer season.
