- Technical Assistance:
The licensing inspector conducted an unannounced renewal inspection Tuesday, June 5, 2018 from 10:20am to 2:00pm. The children were observed playing with Legos, dolls and beads in learning centers. They were also observed singing songs in group time. The infants and toddlers were observed playing on the floor with their teacher. All classrooms were inspected today. The playground and the facility?s mini bus and van were also inspected. Five children?s records and five employee records were reviewed during this inspection. Medication is being administered and was reviewed. A sample of ten injury reports were reviewed at this time. The center's first aid kit and emergency supplies were inspected. Last emergency drill: 05/30/2018 Last shelter-in-place drills: 01/22/2018 Last fire inspection: 01/18/2018 Last health inspection: 05/03/2018 Total enrollment today: 47 Today, the following child to staff ratios were observed: Infants-4:1 Toddlers ?5:1 Two's-12:2 Preschool ?19:2 Pre-K-12:2 School Age ? not present The violations from the previous inspections were checked for corrections. Standard #: 22VAC40-185-350-E-4 Description: Based on observation on 06/05/2018, the center did not ensure that for children from three years to the age of eligibility to attend public school, there was one staff member for every 10 children. EVIDENCE: The staff to child ratio in the Pre-K class was one staff to 12 children. Plan of Correction: Corrected on site. Had staff step in the room to cover the ratio.
Standard #: 22VAC40-185-550-M Description: Based on a review of incident records on 06/05/2018, the center did not ensure that records contained the required information. EVIDENCE: A sample of 10 incident records were reviewed. Out of the ten, three were missing the future action to prevent recurrence, two were missing the date the parent was notified, three were missing the time the parent was notified, and two were missing how the parent was notified. Plan of Correction: Will review with staff today, 06/05/2018.
