Comments:
An unannounced monitoring inspection was conducted from 9:30am - 1:30pm with the center director and staff. There were 72 children, ranging from two months to five years of age, present with 11 staff supervising. Children were observed participating in center play, playing outside, swimming, and eating lunch. Lunch served today was shredded chicken and cheese quesadillas, cucumbers, pineapple and milk. Seven children's files were reviewed and in compliance. Three staff files were reviewed - see supplemental for missing items. The emergency escape drill log was reviewed. One emergency medication and authorization form was reviewed and in compliance. Violations:
Standard #: 22VAC40-185-570-E
Description: Based on observation, it was determined that not all bottles were labeled with the date. Evidence: there were bottles in the refrigerator that were not dated.

Plan of Correction: Staff started dating bottles during the inspection and will continue to do so daily.

Standard #: 22VAC40-191-60-C-2
Description: Based on review of three staff files, and an interview with the director, it was determined that not all staff had Central Registry Search results within thirty days of employment. Evidence: Staff A (Date of hire: 3/15/2018) and Staff C (Date of hire: 4/30/18) did not have documentation of a Central Registry search finding report on file.

Plan of Correction: Will conduct and document follow up for the Central Registry searches that have not returned.

Standard #: 63.2-1721.1-B-3
Description: Based on a review of three staff records, it was determined that no Out of State Child Abuse and Neglect Search was requested by the end of the 30th day of employment. Evidence: Staff C (date of hire: 4/30/18), who indicated that they have lived out of the state of Virginia in the past five years, had no documentation of an out of state search request in the file.

Plan of Correction: Staff was asked to complete the out of state search request during the inspection. Follow up will occur if not received back.
