Comments:
An unannounced monitoring inspection was conducted from 10:00am - 1:00pm with the center director. There were 63 children, ranging from five months to ten years of age, present with ten staff supervising. Children were observed participating in center play to include arts and crafts, games, puzzles, legos and homeliving. Lunch served today was chicken nuggets, baked beans, mandarin oranges and milk. Seven children's records and five staff records were reviewed. The emergency escape drill log was observed and in compliance. Three medications and authorization forms were reviewed and in compliance. Standard #: 22VAC40-185-160-A
Description: Based on review of five staff files, and an interview with the director, it was determined that not all staff had a tuberculosis (TB) test/screening within 21 days of hire or 12 months prior to employment. Evidence: Staff A (date of hire: 5/14/18) did not have documentation of a negative tuberculosis screening on file.

Plan of Correction: Documentation of TB test/screening is now on site.

Standard #: 22VAC40-185-70-A
Description: Based on review of five staff files, and an interview with the director, it was determined that the facility did not have all required information in each staff file. Evidence: For staff A (date of hire: 5/14/18) there was no file with documentation of job title, date of employment, name, address and telephone number for an emergency contact, or two references as to character and reputation as well as competency before employment.

Plan of Correction: File is now on site with complete information.

Standard #: 22VAC40-185-240-D-1
Description: Based on documentation review, and an interview with the director, it was determined that prescription medication was administered by a staff member who did not have documentation of satisfactorily completing a training program for this purpose. Evidence: A child was given two medications on 6/14/2018 by a staff member that did not have documentation of Medication Administration Training (MAT) certification.

Plan of Correction: Documentation of MAT certification is now on site.

Standard #: 22VAC40-185-260-A
Description: Based on a review of documentation, the center did not have an up to date fire inspection report. Evidence: The most recent fire inspection report on file was dated 11/9/2016.

Plan of Correction: Requested a copy of the annual fire inspection.

Standard #: 22VAC40-185-350-E-1
Description: Based on observation, and an interview with the director, it was determined that the facility is not maintaining the proper one staff to four children ratio for children from birth to the age of 16 months. Evidence: There were five children observed in the toddler classroom, with one staff member, and the youngest child was 11 months old.

Plan of Correction: Have parental approval and will obtain written assessment of child social and emotional readiness to be moved to the toddler room.

Standard #: 22VAC40-185-570-E
Description: Based on observation of seven sets of bottles for seven infants, and an interview with staff, not all infant formula was dated: Evidence: Six sets of bottles were not labeled with the date.

Plan of Correction: Will retrain staff on procedures for labeling bottles and will do a safety sweep twice a day to ensure procedures are followed.

Standard #: 22VAC40-191-60-B
Description: Based on review of five staff files, and an interview with the director, it was determined that not all staff had a completed sworn statement or affirmation before employment. Evidence: Staff A (date of hire 5/14/2018) did not have documentation of a sworn statement in their file.

Plan of Correction: I have a sworn statement for the corporation and will complete a new one for La Petite Academy, Inc.

Standard #: 22VAC40-191-60-C-2
Description: Based on review of five staff files, and an interview with the director, it was determined that not all staff had Central Registry Search results within thirty days of employment. Evidence: Staff A (Date of hire: 5/14/2018, did not have documentation of a Central Registry finding on file.

Plan of Correction: I have a Central Registry Search check for the corporation and will resubmit for La Petite Academy, Inc.

Standard #: 63.2-1720.1-B-2
Description: Based on review of five staff files, and an interview with the director, it was determined that not all staff had criminal history record checks prior to their first day of employment. Evidence: Staff A (date of hire: 5/14/2018) did not have documentation of a criminal history record check in their file.

Plan of Correction: I have a criminal history record check for the corporation and will resubmit for La Petite Academy, Inc.
