Comments:
An unannounced Renewal inspection was conducted in the Center on April 25, 2018. Upon arrival in the center, the Licensing Inspector observed 4 children present and in care of 2 staff members. As reported, 10 children are currently enrolled in the center. A sample of 10 records was reviewed, 6 children's and 4 staff records. Documentation of the center's annual fire and health inspections was current. The center does not administer medications. The children were observed during free play, a craft activity, the lunch meal and nap/rest time. One of the parents treated the children to pizza for lunch, the pizza was delivered from a local pizza eatery. Findings were reviewed with Staff at exit. The inspection was conducted from approximately 10:40AM to 3:30PM. If you have any questions, please contact this LI at 540-309-2310. Thank you.

Standard #: 22VAC40-185-130-A
Description: Based on a sample review of children's records, the Center failed to ensure documentation was obtained that each child had received the immunizations required by the State Board of Health prior to the child's attendance in the center. Evidence: One of six children's records reviewed did not contain a current immunization record. The record of child 4. "mom declines" was noted on the child's physical examination form 8/9/17. The child's record noted 3/5/18 as the first date of attendance in the center. There was no documentation of an affidavit from the parent in the record.

Plan of Correction: Staff will review the record with the parent and will request an immunization record or a signed affidavit.

Standard #: 22VAC40-185-160-C
Description: Based on documentation reviewed in a sample of staff records, the Center failed to ensure documentation of TB test/screening was current, at least every two years, from the date of the first initial screening. Evidence: One of 4

Plan of Correction: Staff will schedule a medical appointment to obtain a current TB screening. Documentation will be retained in the staff's record.

Standard #: 22VAC40-185-190-A-3
Description: Based on staff interview and documentation retained in the staff record, the Center failed to obtain documentation to demonstrate that the named Program Director meets program director qualifications as stated in this Standard; 48 semester hours or 72 quarter hours of college credit from a college or university of which 12 semester hours or 18 quarter hours are in child-related subjects and one year of programmatic experience. Evidence: The record for Staff 4. does not contain documentation to support that the individual meets education requirements for program director.

Plan of Correction: Staff will provide documentation to verify qualifications.

Standard #: 63.2-1721.1-B-2
Description: Based on reviewed documentation, the center failed to ensure any applicant or agent involved in the day to day operation had submitted to fingerprinting and had provided personal descriptive information described in subdivision B2 of Code section 19.2-392.02 Evidence: The record for individual 1. did not contain a fingerprint background check. The Virginia State Police CRC Name Search expired 03/27/18.

Plan of Correction: An appointment will be scheduled immediately to obtain a fingerprint criminal background check. Documentation will be retained in the center on file.
