The licensing inspector conducted an unannounced complaint inspection in response to a complaint that was received by the licensing office. Interviews were conducted with staff relating to allegations that a staff member had roughly handled a child and failed to ensure protection to a child in care. The information gathered during the investigation supports a part of the allegation, so the complaint is determined to be valid. Also, the Inspector found the staff did not demonstrate proper behavior guidance to redirect a child's behavior. If you have any questions, contact your licensing inspector at (703) 479-4679.
Standard #: 22VAC40-185-340-A Complaint related: No Description: Based on review of Incident/Accident reports and staff interviews, it was determined staff did not ensure the care and safety of children in the two's class . Evidence: 1. Incident/Accident reports from the two's class were reviewed and various accident reports indicated a child caused the accidents. One set of 5 accident reports took place within 3 days, and staff stated all of these accidents were caused by one child. One accident/incident report described that one child was hit in the head with the bathroom door by another child. Staff stated the child who hit the other child with the door is the same child that caused injuries for 3 other children. Staff stated they have reported the incidents and that the child, as of June 15, is still "acting out" towards children and staff. Plan of Correction: We met with the other child?s parents last Friday morning. We have an action plan in place and will continue to monitor his behavior.
Standard #: 22VAC40-185-400-B Complaint related: Yes Description: Based on record review and staff interviews, staff did not provide behavioral guidance that is constructive in nature and age and stage appropriate to redirect children when they sent a child home early because of climbing on a shelf. Evidence: 1. Staff interviewed stated a child was sent home early because the child would not listen and was climbing up on a shelf. Staff stated this was not safe and so they wrote an incident report and had parent come pick the child up. 2. Interviews also revealed, staff #1 did not try to redirect the child but instead told the Director about the child climbing the shelf. Staff #2 acknowledged she did not try to redirect the child or resolve the behavior and Staff #3 acknowledged they decided to just send the child home. Plan of Correction: We are working with the executive director to compile beneficial trainings in supervision and behavior management for my Beginners teacher team.
