Comments:
A complaint was received by the Division of Licensing Programs Office on 06/11/2018 in regards to allegations of improper diapering practices and failure to prevent the spread of disease. An unannounced complaint investigation was conducted on 06/14/2018. Classroom observations, review of records and staff interviews were conducted on that date. Based on the evidence gathered during this investigation, the complaint was determined to be not valid. Violations of the standards were cited during the investigation that were not related to the allegations. The inspector discussed the following with the program director during the investigation: designated areas for ill or injured children, procedures for children with diagnosed communicable diseases, daily communication with parents, and staffing. The inspector arrived at 12:15 p.m. and departed at 3:30 p.m. Thank you for your time today.

Description: Based on record review, the facility failed to ensure that each child's record contains all components as required by the standards. Evidence: 1. The record for Child #1 did not contain documentation of the name of any additional programs or schools that the child is concurrently attending. 2. The record for Child #2 did not contain documentation of the name, address and phone number of two designated people to call in an emergency if a parent cannot be reached; there were no addressed documented for the two people listed on the child's enrollment form. 3. The record for Child #5 did not contain the following information: documentation of the name, address and phone number of two designated people to call in an emergency if a parent cannot be reached; there was no address documented for one of the two people listed on the child's enrollment form, documentation of the name of any additional programs or schools that the child is concurrently attending, documentation of the written agreements between the parent and the center as required by 22VAC40-185-90, and documentation of the child's previous child day care and schools attended by the child.

Plan of Correction: The Program Director will conduct a quarterly review to ensure updates are obtained as necessary and a detailed review at enrollment.

Complaint related: No
Description: Based on record review and discussion with staff, the facility failed to obtain a search of the central registry within 30 days of employment. Evidence: The record for Staff #2 did not contain a completed search of the central registry. The documented date of hire for Staff #2 was 04/30/2018.

Plan of Correction: The form was submitted in time but not returned within 30 days. In the future, if not received within 30 days follow-up will be done with the Office of Background Investigations.
