Comments:
A Subsidy Health and Safety inspection was conducted on 7/3/18 from 11:55am to 1:35pm to review supplemental health and safety requirements. At the time of entrance there were 34 children ages four months to twelve years old in care with seven staff. Children were observed participating in various activities in the classrooms, and during rest period. Records were reviewed for five children and five staff were reviewed during the inspection. A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection requirements and are identified on the violation notice.

Violations:
Standard #: VENDSUB-000-006-B-11
Description: Based on a review of the facility allergy list and five children's records, it was determined that the vendor did not ensure that a written allergy care plan for each child with a diagnosed food allergy, to include instructions from a physician regarding the food to which the child is allergic to and steps to be taken in the event of a suspected or confirmed allergic reaction. Evidence: 1. The record for child #2 contained documentation from a physician that child #8 was allergic to eggs. However, there was no allergy plan present for child #2 at the facility. 2. The record for child #5 contained documentation from a physician that child #9 was allergic to peanuts However, there was no allergy plan present for child #9 at the facility. 3. Staff #6 reviewed the record for child #2 and child #5, and confirmed that neither child had an allergy plan at the facility.

Plan of Correction: The vendor responded: The parents of child #2 and child #5 will be asked to provide an allergy plan that has been completed by the child's physician. All newly enrolled children with a diagnosed food allergy will have an allergy plan that has been completed by the child's physician prior to starting.

Standard #: VENDSUB-000-012-E-1
Description: Based a review of five staff records, it was determined that the vendor did not ensure that all staff have a current certification in cardiopulmonary resuscitation (CPR) appropriate to the age of children in care. Evidence: 1. The record for staff #3 (date of hire (5/21/18) did not contain documentation for a current certification in cardiopulmonary resuscitation (CPR) appropriate to the age of children in care. There was a CPR certification card, however the certification expired in May 2018. 2. Administrative confirmed there was not documentation for to demonstrate that staff #3 had a current certification in cardiopulmonary resuscitation (CPR) appropriate to the age of children in care.

Plan of Correction: The vendor responded: Staff #3 will complete a course to obtain certification in cardiopulmonary resuscitation (CPR) appropriate to the age of children in care. All new staff will complete a course to obtain certification in cardiopulmonary resuscitation (CPR) appropriate to the age of children in care within 30 days of employment

Standard #: VENDSUB-000-012-E-2
Description: Based a review of five staff records, it was determined that the vendor did not ensure that all staff have a current certification in first aid appropriate to the age of children in care. Evidence: 1. The record for staff #3 did not contain documentation for a current certification in first aid appropriate to the age of children in care. There was a first aid certification card, however the certification expired May 2018. 2. Administrative staff confirmed there was not documentation for to demonstrate that staff #3 had a current certification in first aid appropriate to the age of children in care within 30 days of employment.

Plan of Correction: The vendor responded: Staff #3 will complete a course to obtain certification in first aid appropriate to the age of children in care. All new staff will complete a course to obtain certification in first aid appropriate to the age of children in care within 30 days of employment.
