An unannounced complaint investigation regarding infant diapering and reporting of child injuries was conducted today. The Inspector completed staff interviews and a review of applicable administrative documents today. After careful review of all available information, this complaint was deemed not valid. In addition, an unannounced monitoring inspection was conducted today. There were 58 children in care, with a total of 12 staff members assigned to their direct supervision. Staff records, staff training, children's records, children's medications, emergency preparedness items, background checks, and the physical plant were inspected today. Areas of non-compliance associated with the monitoring inspection from today's visit are noted. The children were observed finishing lunch, playing group movement games, dancing, and taking their afternoon nap during this inspection. The children were served tuna wraps, pineapple, corn, and milk for lunch today. This inspection was completed between approximately 10:20am and 1:15pm. Thank you for meeting with me today. Standard #: 22VAC40-185-60-A Complaint related: No Description: Based on a review of children's records, it was determined that two records were not complete. Evidence: Child record #3 and #5 do not include documentation to show that each parent has reviewed their child's contact information within the past year. Plan of Correction: I will ask each parent to complete a new emergency form.
Standard #: 22VAC40-185-70-A Complaint related: No Description: Based on a review of staff records, it was determined that three records were not complete. Evidence: Staff records #2-#4 do not include two completed reference checks. Plan of Correction: I will complete each reference check.
Standard #: 22VAC40-185-240-A Complaint related: No Description: Based on a review of staff training records, it was determined that three records are not complete. Evidence: There is no documentation of completed First Day Orientation Training for staff #2-#4. Plan of Correction: I will do the orientation for the entire staff group at our next staff meeting.
Standard #: 22VAC40-185-270-A Complaint related: No Description: Based on observations, it was determined that various areas of the center have not been maintained in a clean, safe, and operable condition. Evidence: 1. In the Older Infant classroom, the door to the diaper changing station cabinet has broken and is falling off. Staff have temporarily taped it in an attempt to get it to stay closed. 2. In the 2's classroom, there is a toilet plunger on the floor next to the children's handwashing sink. 3. In both the 2's and 3's bathrooms the floor tiles are very dirty, almost appearing to be rusty. Also in these bathrooms the rubber baseboards are very dirty and beginning to peel away from the walls. 4. In the 4's bathroom, the ceiling light has burned out. There is currently no operating light in that bathroom. Plan of Correction: I will report all of these to maintenance for repair. I have already reported the burned out light in the bathroom.
Standard #: 22VAC40-185-550-E Complaint related: No Description: Based on a review of emergency preparedness drills, it was determined that there is no record to show that fire evacuation drills have been conducted each month. Evidence: There is no documentation of a monthly fire evacuation drill between the months of 12/17-3/18. Plan of Correction: I began employment at this center in April, 2018. We have been conducting and recording monthly evacuation drills since I became the director of this center in April.
