An unannounced renewal inspection was conducted today July 9, 2018 to determine the facility's compliance with licensing standards. The inspector initiated the inspection at 9:05 am and concluded the inspection at 11:50am. The Director participated in the inspection. Upon the inspectors arrival children were observed in a variety of activities. Infants were being diapered with hand washing observed. Infants also played on the matts on the floor with soft blocks and activity centers. Staff engaged in positive conversation and participated in play with the infants. The early pre-K class was divided into two groups with one group outside playing with hula hoops and chalk. The group inside participated in circle time, reviewing the parts of a book, reading a story, singing the weather song and completing the calendar. The two year old group was also observed in circle time with children counting in Spanish and singing songs. The toddler class and pre-K class each had time on separate playgrounds. The toddlers blew bubbles and played with small push toys and trikes. Upon returning to the class the pre-k children played in centers consisting of sensory table with rice, blowing bubbles with straws at the water table, counting and sorting manipulatives and making parachutes at the art table. School age children are participating in the STEM Camp. Today children were working on building roller coaster out of blocks, foam tubing, tape and a marble. Staff explained kinetic energy as children worked to get their marble to roll through the entire roller coaster. Lunch time was also observed. Today's lunch consisted of spaghetti with meat sauce, steamed broccoli, diced peaches and milk. All areas of the facility were inspected to include classrooms, kitchen, hallway, bathrooms and all outdoor playgrounds. The center has age and stage appropriate materials and supplies. Classrooms have children's art work displayed along with photos of children participating in activities. The facility had the following required information posted: the License, last inspection notice, emergency numbers, evacuation maps/procedures, daily schedules, menu and various parent information. The first aid kit and non-medical emergency supplies were checked and found complete for both the building and the four vans used for transportation. The inspector interacted with staff and children in care. Seven staff records and ten children records were reviewed. Medications and medication authorizations were reviewed. Staff on site with MAT certification. Staff have current CPR & First aid training along with daily health observation training. The most recent evacuation drill was documented on 06/25/2018. The two required shelter-in-place drills were completed on 01/31/2018 and 02/13/2018. A lock down drill was documented on 06/25/2018. Violations cited during the previous inspections were check for corrections, there was one repeat violation cited during today's inspection. A complete renewal application will need to be submitted prior to expiration of the license. Standard #: 22VAC40-185-160-A Description: Based on review of staff records, the facility did not obtain documentation of a negative tuberculosis screening for each staff no later than 21 days after employment and shall have been completed within 12 months prior to or 21 days after employing. Evidence: Staff #5, hired 05/14/2018, had documentation of a negative tuberculosis screening however it was dated 02/18/2016 exceeding the requirement to have been completed within 12 months prior to or 21 days after employment. Plan of Correction: Per the Director: Employees must use state provided TB form and not be allowed to submit doctors records to ensure accurate dates. The record was printed within the time frame however the test results had expired. Staff will obtain updated tuberculosis screening.
