Discussed the background check guideline/timelines with the director.

Comments:
An announced initial inspection was completed today. This was completed as part of a change in ownership of the Kiddie Academy at 20775 Century Corner Drive in Ashburn, Virginia which is currently a licensed child day center. The physical plant, 5 staff files, 5 children's files, annual fire and health inspection, staff training, child nutrition, children's activities, medications, emergency procedures, drills and supplies were inspected between 1:00 -3:00pm. There were 73 children under the supervision of 11 staff. During the inspection, children were observed listening to storytime, having tummy time (infants), coloring pictures, creating 4th of July crafts and taking naps. For lunch, the children were served bean and cheese burritos, carrots, fruit and milk. The interactions between the children and staff were seen to be positive and constructive in nature. There are currently 4 staff certified in MAT (medication administration training). The initial application will be complete when the Fairfax Licensing office receives documentation of the change in ownership. At that time, the existing license will be closed and the center will be issued a license under the new ownership. Should you have any questions, please contact me at mary.wizbicki@dss.virginia.gov.

Description: Based on file review, 1 out of 5 staff does not have a search of the central registry completed for a state in which the individual has resided in the preceding five years. Evidence 1. Staff #4 has information on the completed sworn disclosure statement of having lived in another state within the past 5 years. There was no search of the central registry completed for this state.

Plan of Correction: This will be sent out by this Friday, July 6th. If not received within 30 days, the director will follow up on this.
