Discussed the online module MAT offers which includes training to administer rectal medications.

An unannounced renewal inspection was completed today. The physical plant, fire inspection, 5 staff files, 5 children's files, staff training, children's activities, medications, emergency procedures, drills and supplies were inspected between 2:15-4:00. There were 22 children under the supervision of 5 staff. During the inspection, children were observed waking up from naptime, coloring pictures, cleaning up for and eating snack, and enjoying free play. The children bring in their lunches and snacks from home each day, with the exception of Fridays. On Fridays, the center provides pizza for the children. The interactions between the children and staff were seen to be positive and constructive in nature. There are currently 5 staff certified in MAT (medication administration training). Should you have any questions, please contact me at

Standard #: 22VAC40-185-160-C
Description: Based on file review, 1 out of 5 staff records does not have a TB test/screening completed within the past two years. Evidence 1. Staff #5's most recent TB test/screening was completed on 6/17/16.

Plan of Correction: Staff #5 was informed that this needed to be completed in May. Staff #5 does not work over the summer, so she will bring this when she returns in the fall.

Standard #: 22VAC40-185-60-A
Description: Based on file review, 1 out of 5 children's records does not have all required information. Evidence 1. Child #1 has a soy and dairy protein allergy/intolerance documented by the physician on 4/10/18. There is no documentation of what action to take (if any) if the child were to ingest these.

Plan of Correction: The administrative assistant emailed the parent during the inspection, asking to provide this information to the center.

Standard #: 22VAC40-185-340-F
Description: Based on observations, the center failed to keep children under 10 years old within sight and sound supervision at all times. Evidence 1. At 2:33pm, the LI observed child #2 in the hallway near classroom #4. There were no other children or staff in the hallway. After the child told the LI which classroom was hers, the LI opened the door to that room (classroom #4) where she was met by teacher #1. In speaking to child #2 and teacher #1, it was determined that the child was sent into the hallway as a result from her not following directions in the classroom. Teacher #6 stated that she had only been in the hallway for a minute and all of the children were cleaning up to transition to a different class. The teacher stated to the LI that she will not do this again.

Plan of Correction: Administration will discuss acceptable methods for staff to redirect children in the class.

A compliance history is in no way a rating for a facility.

The online compliance history includes only information after July 1, 2003. In addition, the online compliance history includes information regarding adverse actions that may be the subject of a pending appeal. An adverse action is not final until a provider has exhausted or waived all due process rights. For compliance history prior to July 1, 2003, or information regarding the status of pending adverse actions, please contact the Licensing Inspector listed in the facility's information. The Virginia Department of Social Services (VDSS) is not responsible for any errors in or omissions from the compliance history information.
