Technical Assistance:
Consultation on placing the area of the playground off limits due to a hanging branch. Make sure to vary the time of day you conduct the fire escape drills and shelter-in-place drills.

Comments:
A renewal study was conducted today with the director, who was present with 48 children with 10 staff providing supervision. Thirteen children and 3 staff were on a field trip to the zoo and three schoolagers and two staff were at a Junior counselor meeting at corporate office. Three children had medications on site. Six children and staff files were reviewed today. Two buses were inspected as well. Please submit your plan of correction by 7/16/18. Fire inspection: 7/17/17 Health inspection: 6/21/18 Arrival time: 10: 15 am Departure time: 1:45pm

Violations:
Standard #: 22VAC40-185-270-A
Description: Based on observation, it was determined that the outside equipment was not maintained in a safe and operable condition. Evidence: The gate to the back fence near the basketball court was off the hinges and in disrepair. The bolts to the swings were rusted and as a result the intergity of the hardware was compromised.

Plan of Correction: Corrected during visit. During the Licensing Inspection, the maintenance team arrived immediately to repair the fence and added a locking mechanism. All repairs completed before the Inspector completed her visit.

Standard #: 22VAC40-185-280-B
Description: Based on observation it was determined that hazardous substances were not kept locked. Evidence: The Licensing Inspectors observed a spay bottle of sanitizing peroxide and disinfectant in an unlocked cabinet above the sink in the toddler room. The door to the staff lounge was unlocked and on a shelf was a flammable antiseptic solution.

Plan of Correction: Corrected during visit. The spray bottle is located above the teacher station and the unlocked cabinet is approximately six feet off the ground and cannot be reached by toddlers. Staff have been re-trained on the importance of locking up cabinets that contain hazardous substances. Door currently has a lock with a key.

Description: Based on observation, it was determined there wasn't at least 6 inches of mulch in the fall zone of the play equipment on the 3 to 5 year old playground. Evidence: There was 1/2 to 1 inch of resilient surfacing (mulch )surrounding the green and red composite play structure on the 3 to 5 year old playground.

Plan of Correction: Mulch was ordered during the licensing inspector's visit through our maintenance team and will be delivered and placed on Tuesday, 7/17/18.
