An unannounced monitoring inspection was conducted with director and representative from the local agency to investigate a self-reported incident regarding lack of supervision. The inspection was conducted beginning at 12:05 PM and ending at 1:30 PM. Three staff interviews, written and video-recorded documentation were reviewed, and classroom observation were conducted. Parent and child interviews were conducted on June 12, 2018. It was determined that the complaint was valid and a corrective action plan was developed by the center. Description: Based on staff interview and a review of video documentation, the center's staff did not ensure that all children under 10 years of age was always within actual sight and sound supervision. Evidence: Child A was left alone on the center's enclosed playground for a time period of three minutes. Staff A had seven children in her care during the incident.

Plan of Correction: The staff member was given a written reprimand and placed back on a probationary period. The center will provide retaining on sight and sound supervision and face to name identification.
