22VAC40-191 Background Checks (22VAC40-191)
An unannounced monitoring inspection was completed on 07/25/2018. There were 25 children, ages 11 months-5 years, 6 staff providing direct care and supervision present during the inspection. The inspector reviewed 5 children's records and 5 staff records during the inspection. There is no medication being administered at this facility. The children were observed in the following activities: center play, music and movement, TV time, lunch and nap time. Proper hand washing and table sanitization were also observed during the inspection. The inspector reviewed the transportation binder, 6 injury reports, emergency preparedness plans and emergency supplies. The inspector discussed the following with the Program Director: transportation/fieldtrips, nutrition, the outdoor play space, and upcoming inspections by outside agencies. An exit conference was held at the conclusion of today's inspection. Standard #: 22VAC40-185-60-A Description: Based on record review and discussion with the staff, the facility failed to ensure that all children's records contain all components as required by the standards. Evidence: 1. The record for Child #2 did not contain the following information: information regarding allergies and intolerance to food, medication, or any substances and any actions to take in an emergency situation and health information as required by 22 VAC40-185-130. 2. The record for Child #5 did not contain the following information: name of any additional programs or schools that the child is concurrently attending and the health information as required by 22VAC40-185-140. Plan of Correction: The Program Director will request the information from the parents.
Standard #: 22VAC40-185-550-M Description: Based on document review, the facility failed to ensure that written records of children's serious and minor injuries contain the components are required by the standards. Evidence: 1. Injury Record #1 did not contain the time when parents were notified. 2. Injury Record #2 did not contain the time when parents were notified. 3. Injury Record #3 did not contain the time when parents were notified nor did it contain a staff and parent signature or two staff signatures; there was only one staff signature on the report. 4. Injury Record #4 did not contain the time when parents were notified nor did it contain a staff and parent signature or two staff signatures; there was only one staff signature on the report. 5. Injury Record #6 did not contain the time when parents were notified. Plan of Correction: The program director will education the staff that a time needs to be listed and that all signatures are obtained.
Standard #: 22VAC40-185-560-F-4 Description: Based on observations and discussion with the staff, the facility failed to ensure that children under the age of three years are not offered foods that are considered to be potential choking hazards. Evidence: Hotdogs were served for lunch during the inspection. The hotdogs were cut up however there were still large chunks and slices that maintained the circle shape that were considered potential choking hazards for children three years of age and younger. Large chunks of the hotdogs slices and the whole circle slices were observed being served to children in the pre-k class, the 2 year old class and the toddler class that were under the age of 3 years. Plan of Correction: The program director will purchase a hotdog slicer to ensure that the hotdogs are cut into small pieces.
