An unannounced monitoring inspection was conducted today from 10:00am-11:30am. There were 88 children directly supervised by 16 staff. The physical plant, 5 staff training records, 7 children?s records, 4 medications, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed participating in transitioning calmly to and from art and performance activities. There was an abundant supply of materials for the children. The center was clean and organized. Areas of non-compliance are identified in the Violation Notice. Standard #: 22VAC40-185-140-A Description: Based on record review, 2 of 8 children's records reviewed did not contain a physical examination by or under the direction of a physician. Evidence: Child 1 and Child 2's record did not contain documentation of a physical examination by or under the direction of a physician. Plan of Correction: We will ask the parent to bring in a physical examination completed by a physician.
Standard #: 22VAC40-185-50-A Description: Based on observation, not all children's records were treated confidentially. Evidence: The supplemental page from the most recent licensing inspection was posted in the facility. The supplemental page contains confidential information about children enrolled in the program. Plan of Correction: We will take this page down immediately.
Standard #: 22VAC40-185-60-A Description: Based on record review, not all children's records are complete. Evidence: Child 3's record does not contain the playce of employment or a work phone number for the child's father. Plan of Correction: We will ask the parent to fill in the missing information.
Standard #: 22VAC40-185-280-B Description: Based on observation, not all hazardous substances were kept in a locked place using a safe locking method that prevents access by children. Evidence: In the art classroom, the unlocked cabinet under the sink was observed to contain a bottle of bleach spray and two canisters of disinfecting wipes. Plan of Correction: We will lock the cabinet.
Standard #: 22VAC40-185-560-G Description: Based on observation, not all food brought from home was labeled with the child's name and the curent date. Evidence: Six lunch boxes from home were observed without labels. Plan of Correction: We will remind parents to label all food brought from home with the child's name and date.
