Comments:
Licensing inspector arrived at 9:19 am and departed at 12:25pm to conduct a licensing inspection and a Subsidy Health and Safety inspection. At the time of the tour there were thirty-six children in care with 7 staff . Children were observed leaving for field trip, station activities, playing freely, and sleeping . The sample size consisted of 5 children records and 5 staff records. Areas of non-compliance was reviewed with the director. Corrections were not given as of August 8, 2018.

Standard #: 22VAC40-185-160-B
Description: Based on record review, the licensee did not ensure staff had documentation of tuberculosis screening with in the past 2 years. Evidence: During the inspection conducted on July 27, 2018 , staff #5's last TB test was completed on October 6, 2015 which is more than 2 years old.

Description: Based on 1 of 5 records reviewed, the licensee did not ensure the record contained all required documents. Evidence: During the inspection conducted on July 27, 2018, one of child #3 and child #5's emergency contact address was missing city , state and zip code.

Standard #: 22VAC40-185-270-A
Description: Based on observation, the licensee did not ensure the area inside and outside was maintained in a clean, safe and operable condition. Evidence: During the inspection conducted on July 27, 2018, the following was observed outside and inside areas : The outside areas the following was observed: 1-The front porch of the center the four pillar had multiple areas of paint peeling. 2-On the outside play area, the outside door attached to the back classroom is peeling. 3- There was a red roof playhouse had a round hole on the bottom of the entrance into the playhouse on the right with sharp edges and is cracking. 4- The paint on the top of the red barrel stationary equipment and the metal yellow bus stationary equipment were peeling in multiple areas. 5- The metal yellow bus stationary equipment had three bolts that are exposed and can entangle clothing or snag skin. The inside area the following was observed : 6- The hallway toward the preschool classroom, the part of the wall is rough causing to be sharp that is potential of snagging skin. 7- The hallway to the infant classroom, there was dust and dirt on the baseboard that is moveable. 8- There were two chairs a red and blue rocking chair that were at the front of the center the paint is peeling of the chairs. 9- There were multiple areas in the hallway toward the infant classroom that had cracking paint , peeling paint on baseboards,walls and doors.

Description: Based on observation, the licensee did not ensure toilet paper within reach of children. Evidence: During the inspection conducted on July 27, 2018, the restroom near the infant classroom, one of three toilets did not have toilet paper.

Description: Based on observation, the licensee did not ensure children under 10 years of age always shall be within actual sight and sound supervision of staff. Evidence : During the inspection conducted on June 27, 2018, Staff #5 had one child with her when walking to warm up food. The other two infants were sleeping in the room with no actual sight and sound supervision. One infant was sleeping in a bouncy seat and the other child was sleeping in the crib. Staff #5 left the infant room three times.

Description: Based on observation, the licensee did not ensure thirty inches of space between service sides of occupied cribs and other furniture where that space is the walkway for staff to gain access to any occupied crib. Evidence: During the inspection conducted on July 27, 2018, there was a crib being occupied. On one side of the crib there were toys and the other side there was other furniture including rocking chair and toys. There was not a service side that had thirty inches of space.

Description: Based on 1 of 5 records reviewed, the center did not ensure staff records had a central registry finding within 30 days of employment . Evidence: During the inspection conducted on July 27, 2018, Staff #2 ( hire date 4/4/2018) did not have documentation of a central registry findings results.
